Accessing Your Child's Records in My Baptist Chart
Parents/guardians can access their child's health information in My Baptist Chart after setting up a proxy account. As a proxy, you can manage appointments, view lab results, message a doctor, view medications, request prescription refills and more.

To access your child's records, you'll need your own My Baptist Chart account.
Not a Baptist patient or a My Baptist Chart user yourself?
You can still get a My Baptist Chart account to access your child's or dependent's account. First, go to my.baptistchart.com and click the Sign Up Now button. You'll be prompted to enter some personal information to get started. Then, set up your account.
